Harvard Pilgrim Adds Three Additional Hospitals To Their ElevateHealth Network

Harvard Pilgrim has announced the addition of three new hospitals to their ElevateHealth network – Androscoggin Valley Hospital as of April 1, 2015, and Upper Connecticut Valley Hospital and Weeks Medical Center as of May 1, 2015.
ElevateHealth is a partnership of three local non-profits – Dartmouth-Hitchcock, Elliot Health System and Harvard Pilgrim Health Care, working to improve patient experiences and outcomes with the goal of lowering overall health care costs. ElevateHealth focuses on patient-doctor communication, relationships and accessibility, with emphasis on helping patients improve their health and stay well to avoid disease.

- Monadnock Community Hospital
- Alice Peck Day Memorial Hospital
- New London Hospital
- Cheshire Medical Center
- Southern New Hampshire Medical Center

- Valley Regional Hospital
- Cottage Hospital
- Wentworth-Douglass Hospital
- Exeter Hospital
